REQUEST AUDITION FORM

To request an audition pack and a registration form for a specific venue, please email Miss Lewis using the link below.

Please supply the following information on your email:

• Name of Town, Date of Audition and Date of Performance
• Your name, address, including full postcode, telephone number and email address if you have one.

EYB will Email an Audition pack back to you or send you a copy by post.

The Audition Registration fee is £10, payable to English Youth Ballet.

It is advisable to send in the form and cheque to register in advance of the audition day.

Audition information is only available from EYB and not from the venue where the audition will be held.
